• Please fill out your (buyer's) information on the form.
• You can choose to buy a room or rooms for her by checking the boxes on the form. Or, if you want the gift certificate for a specific dollar amount, just let me know in the "questions/comments" section.    
• Also in the "questions/comments" section, be sure to tell me the name of the recipient and if you want the gift certificate sent to a different address.
• Feel free to choose dates or leave blank if you want the recipient to be able to choose her dates. 
• Once you submit the form, I will take care of the rest, including sending the gift certificate and an information package with all the details.
